Documents needed for intake: Proof of income ( examples : W2, last 2 pay check stubs, food stamp card, medicaid card ) Valid Louisiana address Photo ID Social Security Card | Access to Recovery is a Louisiana state grant that provides substance abuse treatment to residence who meet the criteria for care. Treatment includes but is not limited to: Intensive Outpatient Treatment, Outpatient Treatment, Individual Therapy, Family Therapy, and Urine Drug Screens.
